Work-Study and Student Aide Forms

All completed forms for work-study and student aide positions must be submitted to the Office of Student Employment (OSE).

• Student Employment Authorization Contract  2011- 2012 (PDF): required for all new and returning student employees prior to working for an office.
  
  Please TYPE in the highlighted sections and sign and date the bottom. Your supervisor will fill out the supervisor sections and sign and date the bottom.
• Wage Rate form (PDF) Complete the Employee Section and sign and date at the bottom.
• W-4 (PDF) Complete the bottom section of page one. Fill out your personal information - name and address, marital status (boxes 1,3,4) and sign and date the bottom.
• I-9 (PDF) Fill out section 1 of page 4.
• Direct deposit form (PDF) For savings accounts, you must attach a letter from your bank with your name, social security number, account number, and routing number. For checking accounts, you must attach a voided check.

Graduate Assistantship Forms
Forms for graduate assistant positions must be submitted to the dean or director of the department for which the student will be working.
