How do I request my immunization records?

HIPAA privacy regulations require you to complete an Authorization to Disclose Health Information – Immunization Transcript Request Form before NYIT can release your records.

Once completed, this form can be faxed to 516.686.1410.

Records are kept for 11 years after they are received.
