The NYIT Center for Gerontology and Geriatrics -- in partnership with the Long Island Geriatric Education Center at Stony Brook University Health Science Center -- presents a Geroposium:

"Medical Home and the Frail Elderly" on Wednesday, May 27, from 9 a.m. to noon at NYIT's de Seversky Center.

  • New York State Osteopathic Medical Society (NYSOMS)

The "Medical Home" is not a place, but a new and innovative philosophy and approach to comprehensive, patient-centered primary health care. The model -- which is supported by the American College of Physicians (ACP), American College of Family Physicians (ACFP), American Academy of Pediatrics (AAP), and American Osteopathic Association (AOA) -- offers the potential to improve quality of care and quality of life for the frail elderly and other vulnerable groups, as well as the general patient population.

The Geroposium will focus on the rationale and framework of the "Medical Home" approach, the "Guided Care Program" (a prototype for the frail elderly being tested by Johns Hopkins University), the implications of "Medical Home" for education, community service, and research in the medical and allied health fields, and the next steps needed to make the Medical Home for the frail elderly a reality.

This is no charge for this conference and a light breakfast will be served.
